Key Responsibilities

Your role will be central to the smooth operation and coordination of our home personal care services. Key duties include:

  • Assisting the HPC Care Manager with case management, including the coordination of referrals and scheduling of Health Care Assistants (HCAs).
  • Guiding and supporting HCAs in their daily duties to ensure consistent, high-quality service delivery to clients.
  • Coordinating with internal teams and external care partners to implement specific care interventions, such as wound care, medication reminders, and activities of daily living (ADL) support.
  • Educating clients and their families on health management and available community resources to promote better care understanding.
  • Maintaining meticulous and confidential documentation for all client cases to ensure continuity of care.
  • Identifying any service gaps or client needs and reporting these findings to the Care Manager for resolution.

Qualifications

We are looking for a candidate who brings both heart and skill to the role. The ideal profile includes:

  • A minimum qualification of GCE ‘O’ Levels or a Diploma.
  • At least 2 years of relevant experience in eldercare, community health, or social services.
  • A genuine passion for working with seniors and a good understanding of the local eldercare landscape.
  • Excellent organizational, interpersonal, and communication skills; must be a collaborative team player.
  • Resourcefulness and the ability to converse in local languages and dialects is a strong advantage.
